Summary:
|
The Perris Union High School District entered into an Agreement for Professional Services with Sea Change Therapy, LLC for the 2021-22 school year on July 1, 2021. These services provide physical therapy services, including assessment, report writing, Individualized Educational Plan (IEP) services, and attendance at IEP meetings. These services are provided to District students who require triennial assessments, additional assessments, and/or initial assessments according to their IEP.
The term of this Agreement is from July 1, 2021, through June 30, 2022, at a total cost not to exceed $50,000. Due to student needs, the District would like to amend the Agreement to include additional services for students needing a school psychologist and/or other services listed in the Sea Change Therapy Rate Sheet. Additionally, the cost for services will be determined by student needs in accordance with the Sea Change Therapy Rate Sheet, starting at $63 up to $90 per hour. This Amendment will allow for additional staffing services depending on student needs.
The term of Amendment No. 1 to the Agreement with Sea Change Therapy, LLC shall be from November 5, 2021, through the end of the original Agreement, June 30, 2022.
This Agreement is being brought before the Board as ratification due to the immediate need for an additional school psychologist to conduct assessments and provide services for our students.
