Summary:
|
The Perris Union High School District would like to enter into an Agreement for Professional Services with Quality Home Health Agency to provide Specialized Health Services to a Liberty High School student both during bus transport to and from school and in the classroom setting from Monday - Wednesday for the remainder of the 2021-22 school year. The use of a Licensed Vocational Nurse (LVN) is a requirement per the student’s Individual Education Plan (IEP).
The term of this Agreement shall be from September 20, 2021, through June 30, 2022, at a total cost not to exceed $34,560.00.
This Agreement is being brought forth to the Board as ratification due to the immediate need for the LVN services to support the student’s specialized health needs.

The cost for this item is included in the 2021-22 operating budget funded through Special Education, not the General Fund.
